Understanding Hydatid Disease

Hydatid disease is a parasitic infection caused by the tapeworm *Echinococcus granulosus*. It remains a significant socioeconomic problem in developing countries, often linked to improper hygiene and the lack of dedicated veterinary practices. The clinical features are largely non-specific and vary dramatically depending on the location and severity of the disease.

Aetiology & Transmission

The infection is acquired via food contaminated with eggs of the tapeworm. The oncospheres released from the eggs in the bowel enter the portal circulation. Due to this route, the liver is most commonly affected, followed by the lungs. Less frequently, other organs such as bones, the genitourinary system, bowel, and subcutaneous tissues may be infected.

Intracranial Hydatid Cysts

Intracranial hydatid disease is exceptionally rare, comprising only about 2% of all hydatid cases. When they do occur, cerebral hydatid cysts are predominantly located in supratentorial structures within the vascular territory of the middle cerebral artery.

These cysts can be classified into two distinct types:

  • Primary Hydatid Cysts: Occur as a direct invasion of larva that managed to bypass the liver and lung filters to reach the brain. They are usually solitary but can occasionally be multiple.
  • Secondary Hydatid Cysts: Occur as a result of the rupture of primary cysts in other organs, with the contents reaching the brain via embolization. These are usually multiple and lack a brood capsule or scolices.

The World Record: 296 Cysts Removed

Holding the unique distinction of being the only doctor globally with a Guinness World Record in this specialized surgical field, Dr. Prakash Khetan achieved a landmark feat on April 13, 2011.

Operating at Jeevan Jyoti Hospital in Allahabad, India, Dr. Khetan undertook an incredibly rare and dangerous case involving an unprecedented number of intracranial hydatid cysts.

The Surgical Feat: In a grueling, precision-demanding surgery spanning 12 hours, Dr. Khetan successfully extracted 296 hydatid cysts. The extreme fragility of the cysts meant that rupturing even a single one could have caused fatal anaphylaxis or widespread seeding of the parasites throughout the brain. This extraordinary display of endurance and meticulous skill safely removed all 296 cysts, saving the patient's life and etching Dr. Khetan's name into the Guinness Book of World Records.
